PASSPORT INFORMATION
THE FOLLOWING IS REQUIRED WHEN APPLYING FOR A PASSPORT:
1) PASSPORT APPLICATION (forms available in the Phelps County Clerks Office or via the internet at www.travel.state.gov)
(FORMS MUST BE FILLED OUT IN BLACK INK ONLY)
2) ONE INDIVIDUAL PICTURE: Group pictures are not acceptable. Picture must be 2 x 2 in size and on a plain white or off white background. The following locations take passport pictures and may have specifications:

3) PROOF OF CITIZENSHIP
A) Birth Certificate** (Original or certified copy, photo copies not accepted) - must be from the state with the raised state seal on it
B) Naturalization papers - must be ORIGINAL.
C) A fully valid, unexpired passport book or card.

4) PHOTO ID: You must appear in person and submit one of the following identifiers:
A) Drivers License (Permits, Intermediate Licenses, and Out-of-State Licenses cannot count as primary evidence of Identification)
B) Military ID
C) Government or State ID
5) Previously Issued Passport: If you were issued a passport in the past you must present it or fill out a Form DS-64 Statement Regarding a Lost of Stolen Passport.
Failure to do so may cause your passport application to be delayed.
6) Previous Names: If your name does not match your birth certificate you must show why. Example: An original or certified copy marriage license or an original or
certified copy of a court order. If you have had more than one name change you need to show proof of each name change. Photo copies are not accepted.
ROUTINE SERVICE FEES:
Routine service takes 4-6 weeks to get your passport to you and is the best way to go if you are in no hurry to get your passport. Passports will be mailed to the address you put on your application.
ROUTINE ADULT PASSPORT BOOK (16 years of age and older - valid for 10 years) .. $110.00 (payable to the Department of State)
ROUTINE CHILD PASSPORT BOOK (under 16 years of age - valid for 5 years)......................... $80.00 (payable to the Department of State)
ROUTINE ADULT PASSPORT CARD (16 years of age and older - valid for 10 years)................ $30.00 (payable to the Department of State)
ROUTINE CHILD PASSPORT CARD (under 16 years of age - valid for 5 years).......................... $15.00 (payable to the Department of State)
EXECUTION FEE (Acceptance Facility Fee) $25.00 (must be paid in cash only)
ROUTINE PASSPORT BOOK RENEWAL (form DS-82 - if applicant was at least 16 years of age when previous passport was issued, and
passport not expired over 5 years)................................................................. .... $110.00 (payable to the Department of State)
ROUTINE PASSPORT CARD RENEWAL (form DS-82 if applicant was at least 15 years of age when previous passport was issued, and
passport is not expired over 5 years) , . $30.00 (payable to the Department of State)
EXPEDITED SERVICE FEES:
Expediting a passport reduces the processing time at the passport agency. To ensure 2-3 weeks 3 additional fees are required:
EXPEDITE FEE (This is paid to the passport agency to speed up your passports processing time). ..............$60.00 (payable to the Department of State)
OVERNIGHT DELIVERY FEE (This is paid to acceptance agent so your passport will be put in its own express mail envelope and sent overnight to the
passport agency) ................................................ ............... $17.75 (must be paid in cash only)
OVERNIGHT RETURN FEE (This is paid to the passport agency so they will send your passport back to you through overnight
mail***) ............ $12.72 (payable to the Department of State)
*** Passport cards cannot be returned overnight. Therefore, if you are applying for a passport card, you can only speed up the processing time, you cannot ensure the delivery time.
If you need a passport within 2-3 weeks, we recommend applying for the book.
***Passport payments may be paid by either a check or money order made payable to the "Department of State" (effective 3/1/11)
***$25.00 execution fee must be paid in cash only (effective 3/1/11)
***Due to processing time, passport applications will only be accepted between the hours of 8:00 AM - 4:30 PM MONDAY - THURSDAY AND 8:00 AM - 4:00 PM FRIDAY***
ALL PERSONS MUST APPEAR FOR PASSPORT APPLICATIONS TO BE PROCESSED, INFANT AND UP
***For children under the age of 16, consent for passport issuance is required from BOTH parents, or documentation as to why that is not possible. Children must be present with the
parents to complete the application process. Both Parents must submit acceptable identification. **See 4) Photo ID, above** the age for required, routine personal appearance
and signature is 16 years of age.
**For children age 16 and over, consent from ONE parent is required.
